The Uniform Data System is HRSA’s annual, standardized reporting mechanism for health centres. It captures (at minimum) who you served, what services you delivered, what workforce delivered them, what it cost and how it was funded, and key clinical quality measures, using a nationally comparable structure. From a compliance perspective, the “make-or-break” point is that UDS patients, service counts, and clinical metrics are anchored to “countable visits”. If your organization’s definition or operationalization of a countable visit is wrong, every downstream table can be wrong.

For calendar year (CY) 2025 reporting (the cycle most administrators are dealing with as of early 2026), HRSA’s materials consistently communicate a tight annual cadence:

  • Late October: Preliminary Reporting Environment (PRE) opens (early access). 
  • 1 January: the UDS report is available in EHBs / submission period begins. 
  • 15 February: complete and accurate submission due (11:59 p.m. local time). 
  • 15 February–31 March: review period, corrections, and finalization; no changes after 31 March. 

A practical, low-stress way to run UDS is to treat it like a controlled annual close: define scope, lock definitions (patient/visit), validate the main cross-table relationships early, then use the reviewer period to handle true outliers, not fix basic miscounts. HRSA explicitly expects health centres to have systems that collect/organize scope-aligned data and to submit timely, accurate, complete UDS reports.

What UDS is and why it matters

HRSA defines UDS as a standard data set reported annually that provides consistent information about health centres, including: patient demographics; services; personnel; quality of care; cost/efficiency; and revenue sources/amounts. 

HRSA’s own “why we report” framing highlights that UDS data are used to:

  • comply with legislative and regulatory requirements,
  • inform HRSA, Congress, and the public about performance/operations,
  • document programme effectiveness,
  • identify trends over time, and
  • enable comparison with national benchmarks (e.g., Healthy People targets). 

Operationally, UDS is also the backbone for HRSA’s structured monitoring expectations: health centres must be able to produce data-based reports on utilization, patient population trends/patterns, and performance to support internal decision-making and oversight.

HRSA’s current UDS reporting tables publication includes a public burden statement that explicitly ties UDS to:

  • a valid OMB control number (0915-0193, with an expiry date stated in the publication), and
  • mandatory information collection under the Health Center Program authorized by section 330 of the Public Health Service Act (42 U.S.C. 254b). 

This matters for compliance officers because it clarifies that UDS is not “optional performance reporting”; it is an official information collection with a formal control number and stated burden expectations.

The Bureau of Primary Health Care compliance manual chapter on program monitoring and data reporting systems is explicit: a health centre demonstrates compliance by (among other things) having systems to collect and organize scope-of-project data, including UDS elements, and by submitting timely, accurate, complete UDS reports according to HRSA instructions. 

Additionally, the compliance manual’s oversight chapter explains that health centres must comply with programme requirements and award/designation terms, and that HRSA describes when and how it pursues remedies for non-compliance, including enforcement actions. 

Scope of the project as the compliance boundary

HRSA’s scope-of-project guidance defines scope as the approved service sites, services, providers, service area, and target populations. 

UDS reporting is explicitly framed as in-scope reporting: health centres must submit data reflecting all activities in the HRSA-approved scope, and exclude out-of-scope programmes/sites. 

Annual deadlines and reporting workflow

The annual cycle is best understood as a sequence of “locks”: definitions lock first, then data extraction and reconciliation, then final submission, then a reviewer-led correction window.

What is submitted: tables and forms

HRSA’s reporting tables publication (and the accompanying general information fact sheet) describes a structure of 11 UDS tables and three required forms for the Universal Report. 

UDS tables (CY 2025 instrument – table names as published): Patients by ZIP Code; Tables 3A, 3B, 4, 5 (plus Selected Service Detail Addendum); 6A; 6B; 7; 8A; 9D; 9E. 

Required forms: Appendix D (Health IT Capabilities), Appendix E (Other Data Elements), Appendix F (Workforce). 

The Health IT Capabilities form is explicitly stated as something that must be completed and submitted as part of the UDS submission. 

Key definitions that drive correct reporting

  • Patient: A patient is an individual with at least one countable visit (virtual or in-person) in one or more service categories during the calendar year. 
  • Countable visit: A countable visit must meet all fundamental components (licensed/credentialed provider; independent professional judgement; documented services; individualized care, plus the in-person/virtual requirement). If the interaction does not meet the countable visit definition, the individual is not considered a patient for the UDS report and is not included anywhere on UDS. 
  • Calendar-year reporting: UDS is a calendar-year report. Even if your organization operates with a different fiscal year, the reporting period is the calendar year; HRSA’s instructions also note that funding/designation timing affects whether a UDS report is filed for a given year. 
  • Scope of the project boundary: UDS reporting is limited to HRSA-approved scope; out-of-scope programmes/sites must not be included.

The table below is designed as a “data blueprint” for compliance planning—what you must be able to produce, regardless of EHR or health centre size.

Practical compliance checklist with roles and tools

HRSA acknowledges that multiple people typically contribute to UDS and that the lead preparer should organize the team, review activities, and the submission process early. Below is a field-tested way to structure accountability without assuming a particular organizational chart.

The matrix is intentionally written so you can collapse roles (e.g., CFO may also oversee revenue cycle; QI lead may also be UDS lead in smaller centres). The “Lead” column is who should be accountable for completion; other roles are “must consult” or “must approve”.

Common errors, validation checks, and how to avoid them

The most persistent UDS problems are not “data entry mistakes”; they are definition and reconciliation failures that show up as edits, reviewer questions, or implausible trends.

Counting non-countable interactions as visits (or as patients)

If the encounter does not meet the countable visit criteria, the person is not a UDS patient and should not appear anywhere in UDS. Put differently: “services delivered” ≠ “UDS visit.” 

How to avoid: operationalize the countable visit decision tree and validate EHR/report logic against it; treat this as a clinical governance decision, not only an IT problem. 

Submitting without sufficient internal review time

HRSA’s submission checklist explicitly recommends having a complete report ready for internal review at least several days before submission, and it links late preparation to repeated errors year after year. 

How to avoid: create internal deadlines (e.g., “freeze patient/visit counts” date) that precede 15 February.

Weak exception explanations

HRSA’s submission checklist warns that you must address flagged edits through corrections or meaningful explanations; superficial explanations (e.g., “verified with our EHR vendor”) are not acceptable.

Failing to use the Data Audit Report as an iterative control

When a reviewer returns a report, HRSA’s EHBs quick reference instructs health centres to: review comments, run the Data Audit Report, correct/explain findings, and run the Data Audit Report again before resubmission.

How to avoid: treat the Data Audit Report like the final gate in a month-end close, no resubmission without a clean run and documented exceptions.

Non-compliance, corrections, and authoritative resources

Consequences of non-compliance

At the programme level, HRSA’s compliance framework makes clear that it monitors and supports health centres but may pursue remedies for non-compliance (including enforcement actions), and may impose specific award conditions when risk factors exist, explicitly including an organization’s history of timely compliance with reporting requirements.

In the “conditions library”, HRSA includes a specific corrective-action condition for program monitoring and data reporting systems that can require documentation (within a stated timeframe) that the health centre has a system for collecting/organizing data and, where applicable, corrective actions to ensure timely, accurate, complete UDS reporting.

Separately, the U.S. Department of Health and Human Services grant regulations provide remedies for noncompliance, such as withholding payments, disallowing costs, and suspending or terminating awards. 

Corrections, resubmissions, and “appeals” in practice

UDS is designed with a built-in correction pathway:

  • HRSA defines a submission window (1 Jan–15 Feb) and then a review-and-revision period (15 Feb–31 Mar), with final corrected submissions due by 31 March and a freeze after that. 
  • During the review period, an assigned UDS reviewer may send communications and data change requests (per HRSA’s instructions), and health centres are expected to respond within the assigned timeframes. 
  • If a report is returned for corrections, HRSA’s EHB quick reference provides a clear resubmission sequence: review comments, run the Data Audit Report, correct/explain, rerun the Data Audit Report, then resubmit. 

UDS does not function like a courtroom appeal; the operational analogue of an “appeal” is a defensible explanation (supported by internal validation) combined with timely communication with your reviewer, cleared edits, and (when needed) corrected data. 

HRSA also notes that it may grant a reporting exemption under extraordinary circumstances (example given: physical destruction of a health centre), and it specifies that exemptions must be requested directly from BPHC (via the contact form or phone).
