Click Connect is a multi-state post-acute care management organization supporting Skilled Nursing Facilities, licensed providers, and telehealth clinical programs across the United States. Before CapMinds, Click Connect relied on four separate platforms for daily operations: telehealth scheduling, SNF patient data, staff scheduling, and clinical encounter templates. Each tool supported one function, but together they created workflow silos. Staff needed multiple logins. Patient data was separated by facility. Telehealth was disconnected from clinical records. Scheduling lacked state-level timezone, license, and credentialing logic. Clinical templates were static and difficult to customize. The platform consolidated telehealth, scheduling, patient data, provider authorization, clinical templates, reminders, reporting, integrations, and security under one login.
Telehealth Appointment Management
CapMinds replaced the basic telehealth workflow with a real-time appointment queue. The new module includes patient waiting time, appointment status, provider filters, facility filters, station details, device ID, return-call workflow, and multi-provider video support. This gave Click Connect’s teams one live view of telehealth demand and improved coordination between providers, SNFs, and clinical support staff.
Multi-State SNF Calendar
CapMinds built a dedicated SNF calendar that supports all 50 US states with timezone-aware scheduling. Each state row renders appointments in the correct local timezone. Admins can add provider shifts by state, facility, and availability. Scheduling configuration allows teams to define appointment duration and required provider count by time slot. This helped Click Connect reduce cross-state scheduling errors and manage provider capacity more accurately.
Unified SNF Patient Dashboard
CapMinds migrated SNF patient data from PostgreSQL to MySQL 8.0 and created a unified patient dashboard. The dashboard brings together vitals, problems, allergies, medications, immunizations, lab orders, insurance, documents, family history, social history, and encounter data. Click Connect’s teams no longer needed separate facility-level access to view patient context. Clinical and administrative users could work from one consolidated patient record.
Dynamic Clinical Templates
CapMinds replaced static clinical templates with a dynamic template builder. The new module supports text fields, dates, dropdowns, signatures, Yes/No fields, multi-select inputs, rich text formatting, tabbed sections, Group Normals, and Sign & Lock controls. It also includes CPT, ICD-10, HCPCS, CVX, and SNOMED code management. This gave Click Connect more control over documentation structure while improving clinical consistency across encounters.
Management Entity Setup and RBAC
CapMinds built a four-level hierarchy for Click Connect: Management Entity → Practice Entities → Facility Groups → SNF Locations. The platform includes a rules-based authorization engine that checks state licensing, Medicare or Medicaid credentialing, facility-service mapping, and provider authorization before allowing mappings. RBAC allows admins to create roles with View Only or Full Access permissions by module. Provider authorizations can also be auto-disabled when licenses or credentials expire.
Provider Profile and Compliance Management
The provider profile module centralizes NPI, clinical license, DEA license, assigned facilities, services, state-wise licensing, credentialing, collaborating physicians, supervising physicians, availability, leaves, and timesheets. This helped Click Connect manage provider readiness, state eligibility, supervision, and facility assignments from one platform.
Security, Reporting, and Integrations
Continuity EHR includes multi-factor authentication through Email OTP, SMS, Google Authenticator, and Microsoft Authenticator. The platform also includes appointment reports, encounter reports, reminder reports, user reports, active logs, audit trails, CSV exports, Twilio integration, Stripe integration, CCDA export, and medical code management.
